Lifestyle such as diet has shifted from a traditional diet to a western diet with the composition of the foods that contain too much fat, sugar, and contain little fiber found in many foods ready to eat. Based on the incident put Indonesia on the 4th country with the largest number of people with diabetes in the world. This research aims to study and understand in depth about the problems the family nursing care nursing family ineffectiveness against self health management in DM patients in Puskesmas Pucang sewu surabaya. This study uses a case study approach to nursing care by taking two cases as the unit of analysis. The unit of analysis was the patient with diabetes mellitus in the family at the Puskesmas Pucang sewu surabaya. Quantitative data collection is by interview, physical examination and investigation. Data collection instruments used formats nursing care in accordance with applicable provisions of Nursing Campus Soetomo Prodi. Descriptive data analysis. Based on the data analysis we concluded the second assessment disability cases families caring for sick family members that resulted in major nursing problem that arises is the ineffectiveness of the family to self health management in patients with DM. The purpose of family planning and implementation is able to perform health management on a sick family member. Expected for local health professionals to improve the health of patients, especially in providing nursing care to the DM so as to reduce the high incidence of diabetes mellitus.
